Football Equipment Pickup

For those that missed equipment pickup for the past two weeks, we will be holding another sessions during the first week of practice (July 30 to August 3). Please remember that in order to pick up equipment, we must have the following pieces of information (no exceptions).

• Deposit check for $150 dated Nov 30, 2007 (this will be returned to you when the equipment is turned in)
• Final 4th quarter report card (3rd quarter report cards are not acceptable, contact your school's office if you need to get a copy)
• Specific pop warner medical form This form must be signed and stamped by the doctor.
• The player (they need to have their equipment fitted)
• Current picture of player (may already be on file)
• Birth certificate (may already be on file)
• All forms must be signed with insurance information completed (please bring your insurance cards just in case)

Here is the equipment pickup schedule (please try to be there at around 6:30):

• Mon Jul 30th: Tiny Mites (70s)
• Tue Jul 31st: Jr. Mighty Mites (80s)
• Wed Aug 1st: Mighty Mite (90s)
• Thu Aug 2nd: Jr. Pee Wee (100s)
• Fri Aug 3rd: Pee Wee (115s) and Jr. Midgets (130s) Postponed Until Monday August 6
